We are seeking a Patient Care Coach (Registered Nurse) to support disease management programs by providing patient education, follow-up care, and coordination with healthcare providers to ensure high-quality patient outcomes.
🔹 Key Responsibilities:Educate patients on their medical conditions, treatment plans, and self-care practices Conduct regular patient follow-ups to monitor progress and treatment adherence Coordinate with physicians and healthcare teams to support individualized care plans Maintain accurate, complete, and confidential patient documentation Provide ongoing guidance and emotional support to patients and their families Identify and report adverse events, safety concerns, and quality issues Assist with program documentation and required reports
🔹 Requirements:
Bachelor’s Degree in Nursing Valid Kuwait Nursing License Fresh graduates are welcome to apply Driving license preferred but not required Open to all nationalities
